PPE Inspection Checklist Template
The PPE inspection checklist template is the structured check of each PPE item: the condition against the failure signs, the service or expiry date, and the storage. It is the document that catches the silent failure — the harness that held a fall, the respirator with the cracked seal, the hard hat past the service life — because PPE that is not inspected is not protection.

PPE Inspection Checklist — condition, service life, the action
One line per PPE item: the condition, the service life, the storage, and the automatic action. Print the record, export the lines.
| PPE item | Qty | Condition | Service life OK? | Last inspected | Storage OK? | Action | Done? |
|---|
Service life reference: hard hat 5 years (3 if UV-heavy) · safety boots to the manufacturer · FR clothing to the standard · respirator filters by the expiry · gloves to the material · hearing protection to the life · harness 5 years (1 for daily use in many regimes) · first aid PPE per the kit.
Work through it item by item: the category, the quantity, the condition, the expiry, the storage, and the action for the fail. The service-life rules are carried in the template, so the assessor knows the date or the event that retires the item. Print it for the inspection, or export the completed checklist to CSV for your PPE record. The post-incident items are retired, full stop, and the template carries the rule.
How to use the template
- Set the header — the site, the date, the inspector, the next inspection
- Add one line per PPE category — the hard hat, the eye, the hand, the foot, the respiratory, the fall, the hi-vis
- Check the condition against the failure signs — the crack, the delamination, the exposed liner
- Check the expiry or the service date — the date or the event, whichever comes first
- Check the storage — the harness in the sun, the respirator in the chemical store, the storage that degrades
- Record the fails on the replacement list, print it, or export to CSV for the record
Worked example
A PPE inspection: the hard hats, 12 in service, 2 past the 5-year life (replaced), 1 cracked (retired); the welding gloves, 6 pairs, 3 with the liner exposed (replaced); the harnesses, 4 in service, all within 5 years and inspected, 1 that caught a fall last quarter (retired, confirmed); the P100 cartridges, 8 sets, 2 past the change-out (replaced). 6 replacements, all on the list by end of day, the report printed and the CSV exported to the record.
Standard & legal context
The PPE inspection is the maintenance duty under the UK PPE at Work Regulations 1992, which require the PPE to be maintained in an efficient state and efficient order and replaced when damaged or expired. The OSHA 1910.132 general PPE, 1910.136 head, 1910.138 hands and 1910.100 respiratory carry the US equivalents. The manufacturers service lives — the hard hat 5 years, the harness 5-year inspection, the cartridge change-out — are the binding dates, and the template carries them.
Common mistakes
- It looks fine — the PPE is inspected against the failure signs, not the feeling, and the crack and the delamination are the fails
- Keeping the post-incident PPE — the harness that caught the fall is retired, full stop, and the template carries the rule
- One issue date for everyone — the service life runs from the first use or the issue, whichever is earlier, and the roster tracks it
- Storing the PPE where it degrades — the storage is part of the inspection, and the harness in the sun is the fail

The template is the preparation; the sign-off is the professional. Common questions
PPE Inspection Checklist Template — answered
What is a PPE inspection checklist template?
The structured check of each PPE item: the condition against the failure signs, the service or expiry date, and the storage. It is the document that catches the silent failure, and the maintenance evidence behind the PPE at Work duty.
How often should the PPE inspection be done?
Before each use by the wearer, at the site level on a set interval (monthly on the active site, quarterly in the stable workplace), and at the service interval for the items that have one. The template is the site-level check, and the frequency is the site practice.
When must the PPE be replaced?
On the manufacturers service life, after a fall or a single severe event, on the failure signs, or when the inspection fails it. The date or the event, whichever comes first. The template carries the service-life rules, so the assessor knows the trigger.
Does the template carry the service lives?
Yes — the hard hat 5 years, the harness 5-year inspection, the cartridge change-out, the full-face after the chemical exposure. The service-life rules are in the template, and the post-incident items are retired, full stop.
Can I use it in the UK and the US?
Yes — the UK carries the PPE at Work Regulations 1992, the US the OSHA 1910.132 and the specific standards. The template is the inspection structure, and the standard section names both. The failure signs are the same, and the service lives are the manufacturers.
Can I print or export it?
Yes — Print / Save as PDF gives the inspection as a dated report, and Export CSV gives the items, the condition, the dates and the actions for your PPE record. The completed checklist is the document for the file and the one the next inspection opens with.

These templates are practical documents, built to the UK baseline with the US equivalents named on each page. They do not replace a competent person assessment for high-risk work, a statutory assessment, or the advice of your insurer. Where a document has legal force — the fire risk assessment, the COSHH register, the permit to work — the responsible person owns it. Use the template as the preparation, and the competent person as the sign-off.
