The COSHH templates are the document set for the chemical side of HSE: the COSHH assessment form for the substance, the exposure and the hierarchy of controls, the COSHH register for the running record of the significant substances, and the COSHH data sheet for the one-page summary at the point of use. Each one is the COSHH duty as a record, and the three together are the chemical record: the assessment is the depth per substance, the register is the breadth across the site, and the data sheet is the summary at the task.

The Control of Substances Hazardous to Health Regulations 2002 put the duty on the employer to assess the exposure to the substance and to control it, and the assessment runs its own six steps: identify the substance and the exposure, decide who is at risk and how, evaluate the controls that are in place, implement the new controls in the hierarchy, monitor and review. The hierarchy is the heart of it — eliminate the substance, substitute it with the safer one, engineer the exposure down, manage the work around it, and only then the PPE. An assessment that ends at the PPE is an assessment that skipped the first four rungs.
The data that holds the assessment is the exposure. The workplace exposure limit from the EH40 list, the measurement when the limit is in question, the medical surveillance where the substance demands it, and the safety data sheet that is current and in the language the crew reads. The OSHA side runs the same method under the hazard communication standard — the GHS classification, the exposure assessment, the controls — and the document that travels between the two systems is the assessment with the numbers in it.
