COSHH Register Template

The COSHH register template is the running record of the hazardous substances on the site: the substance, the CAS number, the area, the main control, the last review and the next review. It is the document the inspector asks for, the one the health surveillance hangs off, and the one that answers the question when the chemical is the incident and the investigation asks what was controlled, and when it was reviewed.

COSHH Register Template — free HSE template

It is the portfolio version of the COSHH assessment: the assessment is per substance, the register is the site. Add a line per significant substance, with the main control and the review dates, and the register is the document that keeps the chemical side honest over the year. Print it for the file, or export to CSV so it lives in your spreadsheet between the reviews. The review dates are the point, because the register that is not reviewed is the register that is decoration.

How to use the template

  1. Set the header — the site, the owner of the register, the review interval
  2. Add one line per significant substance — the name, the CAS, the area, the main control
  3. Set the last review and the next review dates — the review is the trigger, not the calendar
  4. Score the significant substances where the register carries the score column
  5. Print it for the file, or export to CSV for your system
  6. Review the register at the interval, and on the change — the new substance, the new use, the spill

Worked example

A COSHH register for the workshop: MEK, CAS 109-98-4, paint bay, extract plus procedure plus PPE, reviewed January 2026, next January 2027; the degreaser, CAS 67-63-0, cleaning bay, substitution to the water-based, reviewed March 2026, next March 2027; the welding fume control, the local exhaust, reviewed monthly with the OH; the coolant, the licensed disposal, reviewed annually. Four significant substances, four review dates, and the register is the document the inspector and the health surveillance work from.

Field use of the COSHH Register Template template

Standard & legal context

The COSHH register is the expected output of the UK Control of Substances Hazardous to Health Regulations 2002, which require the assessment and the control of the significant substances, and the register is the record of both. The OSHA HazCom 2012 standard carries the US equivalent, and the GHS classification is the data source. The ISO 45001:2018 clause 6.1.2.3 and the clause 7.5 documentation carry the duty at the management system level, and the register is the documentation the auditor works from.

Common mistakes

  1. One register for the whole organisation — the register is per site, and the substance in the paint bay is not the substance in the warehouse
  2. No review date — the review is the trigger, and the register without the date is the one that drifts
  3. The main control as PPE — the register carries the hierarchy, and the main control is the engineering or the substitution, not the glove
  4. Deleting the retired substances — the history is the record, and the substance that is retired is the line that stays, with the retirement date

Common questions

COSHH Register Template — answered

What is a COSHH register?

The running record of the hazardous substances on the site: the substance, the CAS number, the area, the main control, the last review and the next review. It is the document the inspector asks for, and the one the health surveillance hangs off.

How is the register different from the COSHH assessment?

The assessment is the document per substance; the register is the portfolio of the significant substances across the site. The assessment is the depth, the register is the breadth, and the two together are the chemical record.

Is the COSHH register template free?

Yes — print it, fill it in, and file it. Nothing is uploaded and there is no account. The register is the standard HSE document, and the template is ready to use with the CSV export for your system.

What goes in the COSHH register?

One line per significant substance: the name, the CAS number, the area, the main control, the last review and the next review, and the score where the register carries it. Keep it to the significant substances, and the retired ones stay with the retirement date.

How often should the register be reviewed?

At the set interval, and on the change — the new substance, the new use, the new supplier, the spill that revealed the gap. The interval is the site practice, and the change is the trigger, and both are in the template.

Can I print or export it?

Yes — Print / Save as PDF gives the register as a dated document, and Export CSV gives every column for your spreadsheet or HSE software. The completed register is the document for the file and the one the inspector works from.

Safety disclaimer

These templates are practical documents, built to the UK baseline with the US equivalents named on each page. They do not replace a competent person assessment for high-risk work, a statutory assessment, or the advice of your insurer. Where a document has legal force — the fire risk assessment, the COSHH register, the permit to work — the responsible person owns it. Use the template as the preparation, and the competent person as the sign-off.
